On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form.
Please bring your insurance card and try to arrive at least thirty minutes early for your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.
We have three appointment categories that take up varying amounts of time:
You will be notified of your appointment date and time via an automated system two days prior to your visit. You will have the option to select "no" if you wish to cancel your appointment. If you are unable to make your appointment, please contact our office immediately so we may offer another patient that time.
If you have an urgent medical matter and need to reach a physician, you may contact our office at 469.800.7100, and our answering service will contact the physician on call.
There is a charge of $25 for the completion of forms. An example of forms will include FMLA, Life Insurance Questionnaires or Department of Transportation Assessments. These charges are not billable to insurance, and therefore, it is the responsibility of the patient. Payment is expected at the time of service.
If you require a refill on your non-narcotic medication, please contact your pharmacy and have them fax a refill request to 214.363.2608. This will ensure timely and accurate prescribing information is being relayed for each refill. Please allow 24 hours for all prescription refills. All controlled substances will require a new written prescription. Prescription refill requests will not be filled over the weekend. If you have not seen by your physician in the prior 12 months, your refill requests will be denied.
Patients will be required to sign a Controlled Medicine Contract for chronic use of controlled medications which are potentially addictive and can be abused.

Allergies are problems of the immune system. An allergen is anything that causes the body's immune system to release histamine, one of the chemicals that cause allergy symptoms.
Most allergic reactions happen when the immune system reacts to a false alarm. Normally, the body defends itself against harmful things, such as viruses or bacteria. But the defenses also attack mild things, such as dust, mold, pollen and other environmental triggers at various times of the year, depending on your sensitivities.
We now offer allergy testing at our clinic. Please see our allergy testing & treatment FAQs below for more information.
