Baylor Scott & White Preventive Cardiology and Advanced Lipidology Clinic is dedicated to the study of lipidology and its role in preventive cardiology treatment.

Led by Peter A. McCullough, MD, our goal is to provide advanced heart care through research and education. Preventive Cardiology and Advanced Lipidology Clinic is located on the campus of Baylor University Medical Center, part of Baylor Scott & White Health, in Dallas. Our location allows us to provide convenient access to advanced diagnostic equipment and innovative technology.

Patient Forms

To ensure that your visit to our office is as convenient and efficient as possible, we are pleased to offer our registration forms online. The patient registration form may be completed electronically and printed for better legibility or completed manually.

Appointment Information

Preparing for Your Appointment

Please download and complete our New Patient​ Packet from the Patient Forms section above, and bring the completed forms with you to your visit.

  • Bring all your prescription medications in their current containers.
  • Continue your medications as usual unless instructed otherwise by your physician.
  • Refrain from eating for at least two hours before your visit; the preceding meal should be light, without coffee, tea or alcohol.
  • Wear or bring appropriate clothing such as comfortable, loose-fitting shorts, slacks or jogging attire. Please do not wear one-piece outfits or pantyhose. You may be asked to remove your shirt or change into a patient gown. Bring or wear comfortable shoes designed for walking.
  • Plan to be in our office for one to two hours.
  • Bring your insurance cards.

If you need medical records transferred to us BEFORE your visit, please call our office for information on obtaining the transfer request.
