At the Baylor Scott & White Senior Health Center – Plano, you will find the utmost in comfort and convenience. Our team of providers is committed to providing compassionate care and improving senior health in Plano and across the entire Metroplex, from osteoporosis and memory loss to diabetes and hypertension.

We are conveniently located in Plano with centers equipped to deliver primary care by teams of physicians specialized in geriatric medicine.

The Baylor Scott & White Senior Health Center – Plano serve patients 55 and older with selected insurances and 65 and older with Medicare Parts A and B. The team of providers at each center includes dietitians, social workers, and chaplains who can help seniors and their families understand and assist to changing health needs.

Baylor Scott & White Senior Health Center – Plano accepts traditional Medicare and has limited participation in some Medicare Advantage health plans. We also accept Scott & White Health Plan, Humana and Aetna.

Appointment Information

Baylor Scott & White Senior Health Center – Plano offer comprehensive care for the senior citizens of Plano and across the entire Metroplex. Our team of physicians and staff understand the special medical needs of seniors and provide the healthcare services you need.

We welcome new patients and can often accommodate same day appointments.

Making an Appointment

New patients receive a thorough assessment that includes a variety of tests for conditions that can affect us as we age including memory loss, osteoporosis, hypertension and incontinence.

On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form. Please bring your insurance card and try to arrive at least 30 minutes early for your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.

We have three appointment categories that take up varying amounts of time:

  • Annual exams or new patient visits—Plan on spending one hour including check-in time, filling out forms and the actual physician visit
  • Follow-up exams—15 minutes
  • Sick visits—Varies depending on the illness, but plan on at least 30 minutes

Canceling an Appointment

Our appointments are confirmed by an automated service. Please listen carefully and follow the prompts to either confirm or cancel your appointment. If you later decide that you are unable to keep your appointment or you are going to be late, please call your physician’s office as soon as possible. This courtesy allows us to be of service to other patients.

Emergency Care

If you have an urgent problem, you may call your physician’s office to schedule an appointment. However, if you feel it is a life-threatening emergency, call 911. Do not delay by calling the doctor’s office first.

Confidentiality

At BSW​ Senior Health Center, we have a strict patient confidentiality policy. Your medical record is strictly private and will not be shared with your employer, friends or relatives without your written permission.

MyBSWHealth is an online tool where you can communicate with your providers, schedule an appointment, access and manage your family’s health.

Beginning in 1998, Baylor HouseCalls has been proud to provide primary care to older adults who are home-bound and unable to access health care through regular office visits with a physician.
