The lead physician here, Matthew Berchuck, MD, specializes in the treatment of a variety of spinal conditions with a strong focus on cervical, thoracic and lumbar spine surgery. Board-certified by the American Board of Orthopaedic Surgery and dedicated to delivering quality spine care, Dr. Berchuck has many years' experience in the latest advances in spinal care that include up-to-date, minimally invasive spine surgery technique.

Located in the heart of downtown Dallas, TX, on the campus of Baylor University Medical Center, part of Baylor Scott & White Health, BSW Spine Surgery Center offers advanced digital imaging capabilities and an electronic health record system in an inviting environment.

To ensure that your visit to our office is as convenient and efficient as possible, we are pleased to offer our registration forms online. The patient registration form may be completed electronically and printed for better legibility or completed manually.

Appointment Information

Making An Appointment

You may schedule your appointment using our online capabilities or by calling our office directly.

On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form.

Please bring your insurance card and try to arrive at least 30 minutes early for your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.

Prescription Refills

The Baylor Scott & White Spine Surgery Center ​is pleased to serve our patients utilizing an electronic health record (EHR).

Prescriptions may be filled electronically, or your pharmacy may fax requests for prescription refills to 214.820.1351.
