Gain confidence with virtual diabetes management

Personal health coaching from Baylor Scott & White serves as an extension of the care from your provider and gives you a digital solution when managing diabetes feels challenging. The program is free of charge to you.

If you have been diagnosed with Type 2 diabetes, virtual diabetes management connects you with support between visits with your primary care provider, so you can move toward effective blood sugar control.  

How virtual diabetes management works

Your virtual diabetes coaching journey can begin in several ways:

  • Your provider recommends you request an appointment with a health coach.
  • You have seen information about this program on a poster or informational card in your provider’s office and want to request an appointment.

You can request an appointment with a coach at a time that fits your schedule. When you have your first meeting with your coach, they’ll spend time getting to know you and, talk with you about your goals and lifestyle, and work to create a personalized plan.

Your provider will stay connected to your coaching sessions with updates and a summary of what you accomplished at the end of your program.

  • Virtual coaching sessions on your schedule

    Based on your needs, you and your coach will determine how often you will meet for a 30-minute virtual coaching session by phone or video.

  • Goals created by and for you

    You and your coach will set goals based on your daily habits and routines that will help you better manage diabetes and improve your overall health.

  • Get support and guidance through secure messaging

    Your coach will send you a message through your MyBSWHealth app on the weeks that you don’t have a virtual coaching session to recap the goals that you have set together, as well as provide information and education.

    You can also communicate directly with your coach via secure messaging on MyBSWHealth.

  • Continued support after the program ends

    Support doesn’t end once you complete the program, which typically lasts six months. You can re-enroll in the program if you want to continue working with a coach.

Meet your virtual diabetes wellness coaches

The BSW Personal Health Coaches help you and your physician have the information and tools needed to understand diabetes and make behavior changes.

Our coaches are experienced in diabetes coaching. They provide support and education validated by the Center for Disease Control (CDC), the American Diabetes Association (ADA) and Baylor Scott & White Health.
