​​​​​​Bridges to Excellence® (BTE) is a family of programs designed to reward recognized physicians, nurse practitioners and physician assistants who improve healthcare quality and value by implementing comprehensive solutions in the management of patients, and delivery of safe, timely, effective, efficient, equitable and patient-centered care. BTE programs measure health outcomes, reduce preventable care defects, promote a team-based approach to caring for patients, realign provider payment incentives around quality and reward excellence for significant leaps in the quality of care.

Seeking care from a BTE-recognized provider means you are receiving the highest quality care and reducing your financial and patient burden of unnecessary hospitalizations and complications due to your disease.

Diabetes Care

More than 22 million Americans have either Type 1 or  2 diabetes, and millions more are yet undiagnosed. Almost two million Texas residents over the age of 18 have been identified with diabetes. If you fall into this group, Baylor Scott & White Medical Center – Austin/Round Rock is proud to announce its participation in the Bridges to Excellence (BTE) Care Recognition Program.

What sets these providers apart?

Eligible clinicians voluntarily apply for BTE recognition by submitting information on how they comprehensively treat and manage their patients with diabetes, and work in concert with other medical specialists in regard to:

  • Blood pressure (BP) control
  • LDL cholesterol control
  • HBA1c control
  • ​Ophthalmologic exam
  • Nephropathy assessment
  • Podiatry exam
