Designed for those with specific heart conditions, like heart failure, you’ll have access to a care team that helps you monitor your health and provides you and your caregivers support when you’re at home.
Our goal is simple: to give you better support over your heart condition and to avoid unnecessary trips to the emergency room or even hospitalizations.
Our services
You see your doctor regularly, but what about the time between appointments? Through Connected Care, the daily vitals readings that you take from the comfort of your home are sent directly to both your provider and your Cadence Care Team. In addition to reaching out, if any of the daily vitals' readings indicate a potential concern, the Cadence Care Team provides regular telephone-based check-ins. The program is designed to help you feel better day-to-day by:

How it works
Remote monitoring, using simple devices at home, helps you stay connected with your provider. Take the first steps toward better cardiovascular care:
Talk to your provider and enroll
Your provider will recommend the program if they think it could have a positive impact. A Cadence Care Team member will help you complete the enrollment.
Check-in daily
You will receive two devices: a weight scale and a blood pressure cuff to monitor your vitals daily. Both devices provide check-ins to your team.
Update your team automatically
Your vitals readings will be sent automatically (no extra work required) and securely to your Cadence Care Team and provider.
Receive personalized support
Your team will check in regularly through telephone visits. If any of your readings indicate a potential concern, you will be contacted with the help you need.

How do I sign up?
If your provider believes you may benefit from the program, they will refer you and the Cadence Care Team will reach out with more information to help you enroll.

Yes. Your provider will continue to manage your care, working closely with your Cadence Care Team to ensure the best possible support.

Who makes up my Cadence Care Team?
Your Cadence team is made up of NPs, RNs, patient support and clinical navigators.

Connected Care is a covered benefit under most insurances. The team will review your health plan coverage during your enrollment call.
