Located within our Health and Wellness Center, the new and modern Family Health Center provides primary medical care services in a comfortable and welcoming atmosphere to residents from the community. The Family Health Center is operated by a team of healthcare professionals that includes a physician, nurse practitioner, behavioral health therapist/social worker, diabetes educators, registered dietitians and a referral coordinator.

We offer a collaborative, team approach to caring for patients and work together to combine services in one visit, reducing the number of visits needed for care. This patient-centered practice ensures patients and members receive comprehensive care, specialty care and access to supporting wellness services.

Patient qualifications for services

Our expertise is based on the knowledge of our patients in the context of their community. This community has a very high incidence of uninsured residents, many of which suffer from chronic diseases such as diabetes.

To receive medical services, you must bring:

  • Proof of residency (driver’s license, lease agreement or utility bill)
  • Proof that family income does not exceed 200% of the federal poverty level (check stubs, income tax return or letter from employer)
