Patient Forms

To ensure that your visit to our office is as convenient and efficient as possible, we are pleased to offer our registration forms online. The patient registration form may be completed electronically and printed for better legibility or completed manually.

Appointment Information

Making an Appointment

Our hours of operation are Monday through Friday 8:30 a.m. to 5 p.m. for all locations other than our Plano office, which is 9:00 am to 5:00 pm. You may schedule your appointment using our online capabilities or by calling our office directly. Please note that only routine appointments should be made online.

On your first visit, we will begin a permanent record. Please bring any medications you are taking so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form. Please bring your insurance card and try to arrive at least 30 minutes early for your first visit. If you have been under the care of another doctor, we ask your permission to transfer some of your records.

Prescription Refills

Surgical Institute is pleased to serve our patients utilizing an electronic health record (EHR). Prescriptions may be filled electronically or your pharmacy may fax requests for prescription refills to our Dallas office: 469.800.7570, Plano office: 469.800.5241, McKinney office: 469.800.5388 or our Carrollton office: 469.800.1225.

In Case of Emergency

If you have an urgent problem, you are welcome to call our offices to schedule an appointment. We will do our best to accommodate your request. However, if you feel it is a life-threatening emergency, call 911. Do not delay by calling the doctor's office first.

Surgical Treatments and Procedures

Baylor Scott & White Surgical Institute has maintained a long-standing reputation for quality surgical care throughout the Dallas and Plano communities. Our surgeons are known for consistently offering patients comprehensive care in a comfortable and caring environment.

In an effort to offer our patients the best outcomes possible, we take an individualized approach to developing treatment plans that include minimally invasive surgical techniques where appropriate. Some of our procedures include:

  • Advanced laparoscopic surgery: Laparoscopy is a technique for doing surgery using a thin, lighted tube with a camera called a laparoscope. The scope is put through one small skin incision, and surgical tools are put through other small incisions. Many surgeries are done using this technique. Some of the more common general surgeries we perform using this method are Gallbladder, Hernia, and Abdominal Surgery.
  • Pancreas, liver, gastrointestinal and endocrine surgery
  • Head and neck surgery: Our surgeons perform head and neck surgeries including thyroid, parathyroid, and parotid. Surgery can be an effective treatment for hyperthyroidism, malignant nodules or those that grow or cause symptoms.

Conditions Treated

Appendicitis

The appendix is a hollow structure about the size of your little finger and opens off the colon (large bowel). The purpose of the appendix is unclear, but if it is blocked, it may become infected. Your side may hurt so much that you called your doctor or when straight to the emergency room. If the symptoms came on quickly, you may have appendicitis.

Breast Cancer

Having breast cancer means that some cells in your breast are growing abnormally. Learning about the different types and stages of breast cancer can help you take an active role in your treatment.

Colon Cancer

Cancer occurs when cells in the body begin changing and multiplying out of control. These cells can form lumps of tissue called tumors. Cancer that forms in the colon is called colon cancer. 

Gallbladder Disease

Also referred to as cholecystitis is an inflammation of the gallbladder wall and nearby abdominal lining. Cholecystitis is usually caused by a gallstone in the cystic duct which connects the gallbladder to the hepatic duct.

Hernia

A hernia (or rupture) is a weakness or defect in the wall of the abdomen. This weakness may be present at birth or it can be caused by the wear and tear of daily living. If left untreated, a hernia can get worse with time and physical distress.

Pancreas Disorders

Many disorders of the pancreas require clinical care by a physician or other healthcare professional. Some of the more common disorders include pancreatitis, pancreatic cancer, and cystic fibrosis.

Thyroid Disorders

Many disorders of the thyroid require clinical care by a physician or other healthcare professional.
