What is a STEMI?

ST-segment elevation myocardial infarction (STEMI) is a type of heart attack. It occurs when a blockage stops all or nearly all blood flow through an artery leading to the heart. Left untreated, a STEMI can cause long-term complications and can be fatal.

STEMIs get their name from how they appear on an electrocardiogram (EKG). The ST segment is a pattern of electrical signals in the heart. On an EKG, the ST segment is typically flat, but a complete or nearly complete blockage causes it to elevate.

STEMI vs. NSTEMI heart attacks

STEMI and NSTEMI (non-ST-elevation myocardial infarction) are both types of heart attacks. While a STEMI occurs when an artery is completely blocked, an NSTEMI occurs due to a partial blockage or temporary interruption of blood flow.

The two heart attacks also show up differently on an EKG. The blockage causing a STEMI creates a spike in the ST segment, which is visible on an EKG. An NSTEMI doesn’t cause a spike in the ST segment. EKGs usually show no changes in the heart’s electrical activity or a depressed ST segment, and doctors may need to order other tests to determine whether someone is having an NSTEMI.

Symptoms of STEMI

Symptoms of a STEMI are generally very typical heart attack symptoms. However, other symptoms are possible. Recognizing these symptoms and seeking emergency medical attention can help protect your heart from additional damage or even save your life.

Symptoms of a STEMI heart attack include:

  • Chest pain or pressure that may radiate to the neck, arm or shoulder
  • Excessive sweating
  • Fatigue or weakness
  • Nausea or vomiting
  • Shortness of breath
  • Upper abdominal pain

STEMI causes

STEMIs occur when the heart doesn’t receive enough oxygen to function. With a STEMI, oxygen deprivation is caused by a blockage preventing oxygenated blood from reaching the heart.

Blockages that cause a STEMI heart attack are typically related to plaque formation. Plaque is a substance made of cholesterol, fat and other materials, and it can build up along artery walls. When you develop plaque buildup, your doctor may diagnose you with atherosclerosis.

Pieces of plaque can break off and create blood clots, which can block the coronary arteries and lead to STEMI.

STEMI risk factors

Many health conditions and lifestyle factors can increase your risk for STEMI and other types of heart attacks. Unfortunately, you have no control over certain heart attack risk factors. These include age, sex, your family history and your race. However, your lifestyle also plays a large role in your risk, and those factors are well within your control.

Risk factors for STEMI include:

  • Age: Your risk increases as you grow older. On average, STEMIs occur at age 65 or later.
  • Family history: Your risk is higher if your parents had heart disease.
  • High blood pressure: Long-term high blood pressure damages blood vessels, causing scars where plaque may collect and slow blood flow.
  • High cholesterol: Too much low-density lipoprotein (LDL) cholesterol can build up inside your arteries and form artery-clogging plaque.
  • Obesity: Excess weight is linked to other STEMI risk factors, such as high blood pressure, high cholesterol and diabetes.
  • Pre-existing health conditions: Unmanaged chronic conditions, such as diabetes, kidney disease and rheumatoid arthritis, increase your heart attack risk.
  • Sex: Males have more heart attacks than females.
  • Smoking: Tobacco use contributes to plaque buildup, increases blood pressure and cholesterol and thickens blood.

Complications

Left untreated, a STEMI heart attack can be fatal. Even when treated properly, this type of heart attack can leave lasting damage to the heart muscle. As a result, you may experience complications, including:

  • Arrhythmias: Injury to your heart’s electrical system can result in an abnormal heartbeat. Your heart may beat irregularly or faster or slower than normal.
  • Heart failure: Your heart loses its ability to pump enough oxygen-rich blood to your body to maintain optimal health.
  • Mitral valve regurgitation: A STEMI can weaken or damage the heart muscle that deforms the mitral valve, causing blood to flow backward.

Diagnosing STEMI

STEMIs are medical emergencies usually diagnosed and initially treated in an emergency room. When you arrive in an ER, your medical team will perform a clinical evaluation and order diagnostic imaging and blood tests to determine which type of heart attack you’re having.

If these steps do not indicate a STEMI, you may have more tests that help doctors find out what’s causing your symptoms.

Treatment for STEMI

Treating a STEMI heart attack aims to restore blood flow to the heart. Doing this quickly helps protect against further harm to the heart muscle. You may benefit from multiple simultaneous treatment approaches. No matter what treatment your team prescribes, it is most useful when administered quickly. This underlines the need to seek medical care as soon as you recognize heart attack symptoms.

Surgery

An interventional cardiology procedure called angioplasty and percutaneous coronary intervention (PCI) is commonly used to treat STEMI. This procedure must occur within 12 hours of your first symptom and involves the insertion of a thin tube (catheter) through a vein in your wrist or groin. A balloon at the tip of the catheter is inflated to clear the blockage, and a stent is placed to keep the artery open.

Depending on the severity of your blockage, you may benefit from coronary artery bypass grafting (CABG). Also known as bypass surgery, this open-heart procedure reroutes blood around severely blocked arteries.
