Making an Appointment
We are open Monday through Thursday between 9:00 a.m. and 5:00 p.m. and Friday from 9:00 a.m. to 12:00 p.m. We are closed 12:00 p.m. to 1:00 p.m. for lunch.
Appointment slots are reserved daily for acute illnesses and minor emergencies. We are a specialist office and tend to be booked out farther than your primary care physicians. We will make every effort to fit you in as quickly as possible.
We request that new patients arrive 15 minutes prior to their scheduled appointment to complete their necessary insurance and medical history forms. Our patient forms can be found in the patient forms section.
What to Bring to Your Appointment
In order to make your appointment as efficient and beneficial as possible, we encourage you to bring:
- Your insurance card
- A list of current medications (including over-the-counter medications, supplements, and vitamins). Many patients find it easier to actually bring a bag containing their medications.
- Pertinent information about your medical and surgical history. This includes old medical records, lab or imaging tests.
Cancelling an Appointment
We have an automated service that will remind of your appointment the day before your appointment. If you are unable to keep your scheduled appointment, please call our office at 469.800.7700 at least 24 hours in advance to cancel or reschedule. This is important in helping us provide continuous care to all of our valued patients, and we appreciate your cooperation.
Prescription Refills
We are pleased to serve our patients utilizing an electronic health record (EHR).
Prescriptions may be filled electronically or your pharmacy may fax requests for prescription refills.
After Hours
If you need assistance during non-office hours, the on-call physician can be reached through our answering service by calling our office at 469.800.7700. The on-call physician will contact you and attempt to assist you with your problem. If you believe you have a life-threatening emergency, please call 911 or proceed to the nearest emergency room.
