If you as a patient find a third-party application (mobile app) that you would like to use to access and import your health information, FollowMyHealth provides the ability for you to do so. Allowing Access will let third party apps access your personal health information (PHI), such as allergies, immunizations, vitals, test results, medications, procedures and conditions.
Setting up third party application access:
1. Navigate to third-party app of choice and follow steps for account creation.
2. Within the third-party app, search for BSWH hospital location and indicate that you want to retrieve your records.
3. You will be redirected to your BSWH - FMH login screen to enter your credentials (this is a safety feature - you will never be asked to enter your FMH credentials within the external app).
4. For technical assistance with viewing or disconnecting access, please contact the third-party app.
Warning: Sensitive External Data Access Approval
By allowing access to FollowMyHealth® (FMH) and third-party apps, you are authorizing a third-party application to have continued access to your personal health information or the personal health information of others with whom you have proxy rights. FMH and third-party applications are NOT provided by Baylor Scott & White (BSWH) or its affiliates, and therefore, the application or the third-party creator of the application may have different privacy and security protections. However, allowing FMH and the third-party access to access your information is your personal choice. You should carefully review the legal Terms and Conditions, including any terms relating to protecting your information, as it relates to FMH and the third-party application before authorizing BSWH to allow FMH and the third party to access your health information. Please note, once you authorize this access, it remains until you revoke the authorization. Lastly, if you wish to revoke FMHs or the third party application’s access authorization, you must go to FMH or the third-party application to revoke access authorization. The revocation will not affect any data provided before revocation.
