Pre-Register for Your Appointment

We offer several options to pre-register for your scheduled upcoming appointment.  Pre-registering for your appointment shortens the check-in process at the facility and provides you information about your benefits and an estimate of your cost of care prior to service. It only takes 10 to 15 minutes to complete. Follow the guidelines below to expedite the pre-registration and appointment experience. Watch the following video to understand more.

Please note if you do not initiate the pre-registration process with us, a representative from our Pre-Registration Department may call you to obtain the necessary information for the registration process.

All information is secure and confidential.
