What is prostate cancer?

Prostate cancer is the uncontrolled growth of abnormal cells that form in the prostate. The prostate is a small gland in a man’s reproductive system that helps produce semen. It sits just below the bladder and surrounds the urethra, the tube that carries urine from the bladder outside the body. 

Most prostate cancers occur in men who are older than 65—the average age at diagnosis is 67. It is the second-most common cancer in men, behind skin cancer. The good news is that prostate cancer can often be cured or controlled, especially if it’s found and treated early.

Types of prostate cancer 

Most prostate cancers are adenocarcinomas. These cancers begin in gland cells, including the cells in the prostate that produce and secrete fluid. 

In rare cases, prostate cancer can develop from other types of cells. These less common forms of prostate cancer include: 

  • Small cell carcinomas
  • Transitional cell carcinomas
  • Neuroendocrine tumors
  • Sarcomas

Symptoms of prostate cancer 

Prostate cancer usually does not cause symptoms in its early stages. You may develop symptoms only when the condition becomes advanced, which is why prostate cancer screenings are so important.

Symptoms can develop when the prostate grows. Because of where the prostate is located, it can start to interfere with urinary function. As the condition progresses, it can spread (or metastasize) to other parts of the body, causing bone pain, fractures, anemia and urinary incontinence. It’s important to remember, though, that these symptoms of prostate cancer can also occur with other noncancerous conditions. 

Prostate cancer symptoms can include: 

  • Blood in the urine or semen (hematospermia)
  • Burning or painful urination (dysuria)
  • Difficulty starting your urine stream
  • Erectile dysfunction (ED)
  • Frequent urination at night
  • Needing to urinate immediately
  • Painful ejaculation
  • Recurring urinary tract infections
  • Urine dribbling after finishing using the restroom
  • Weak urinary stream

What causes prostate cancer? 

Medical experts do not know exactly what causes prostate cancer. The condition occurs when healthy cells develop genetic changes, or mutations, that interfere with normal functioning, but it’s unclear what causes those changes.

In some cases, you inherit genetic mutations from your parents. For example, mutations in the BRCA1 and BRCA2 genes, which are linked to breast cancer, are also linked to prostate and a few other cancers. These genetic mutations are often inherited and may account for about 10% of prostate cancers. Genetic testing can help you learn if you have these inherited gene mutations. 

Your genes can also change over the course of your life. Known as acquired genetic mutations, these are the changes most linked to prostate cancer. Some research suggests that high levels of certain hormones, exposure to radiation, lifestyle choices and some cancer-causing chemicals can cause genes to mutate. However, more studies are needed to understand whether these links exist and exactly how they work. 

Prostate cancer risk factors 

You don’t always have control over your risk for prostate cancer. Although some research suggests that diet can play a role in the condition, the evidence isn’t very strong. Many known risk factors for the disease are beyond your control. 

Risk factors for prostate cancer include: 

  • Age: Your risk of developing prostate cancer increases as you get older.
  • Family history: Having a first-degree relative who has had prostate cancer increases your risk, particularly if the relative was diagnosed before age 65.
  • Race: Black men are more likely than other men to develop prostate cancer. They are also more likely to develop the condition at a younger age, and experience more aggressive forms of the cancer.

Screening for prostate cancer

Screenings are important for detecting prostate cancer early. That’s why men should have informed conversations with their primary care providers about the benefits of screening and when to begin.

The American Cancer Society recommends having those conversations:

  • At age 50, for men at average risk of prostate cancer expected to live for 10 or more years
  • At age 45, for men with a high risk of prostate cancer, including Black men and men with a father or brother diagnosed with the disease before age 65
  • At age 40, for men with multiple family members diagnosed before 65 

If screenings suggest the possibility of prostate cancer, additional tests or procedures may be required. 

Prostate screening tests include: 

  • Digital rectal exam (DRE): In a DRE, your doctor will insert a gloved, lubricated finger into your rectum to examine your prostate. Additional tests may be necessary if any texture, shape or size abnormalities are found.
  • Prostate-specific antigen (PSA) test: This blood test measures the level of PSA, a substance made by the prostate gland. While a small amount of PSA in the blood is normal, higher levels may indicate infection, inflammation, enlargement or cancer. 

How is prostate cancer diagnosed? 

Some of the tests used to diagnose prostate cancer are the same as the tests used to screen for the condition. Often, the process begins with a physical exam and a discussion of your symptoms. Your doctor may order a prostate-specific antigen (PSA) test, a blood test that detects high levels of PSA, which can be a sign of prostate cancer. Based on the results, you may need a biopsy to confirm your diagnosis. 

Medical history and exam

Your primary care provider will ask you about your symptoms. They may also ask about your family history of prostate cancer and discuss whether you have any other risk factors for the condition.

If your provider suspects prostate cancer, they may perform a digital rectal exam (DRE). They will insert a gloved, lubricated finger into your rectum and feel for lumps or other abnormalities that prostate cancer may cause. 

Prostate cancer stages

After you are diagnosed with prostate cancer, your oncology team will give your cancer a stage. Prostate cancers are staged from a level of I to IV, and the stage helps your cancer team determine how to treat your cancer as effectively as possible. 

Your cancer’s stage is based on whether it has spread within the prostate, to your lymph nodes, or to your bones and other organs. Your team will use the results of your PSA test and the Gleason score or Grade Group given by your biopsy results, as well as other tests, to determine the stage. Those tests include: 

  • Imaging tests: You may have a bone scan, positron emission tomography (PET), MRI or CT to see if cancer has spread outside the prostate.
  • Biopsy: Your doctor may take samples of pelvic lymph nodes or of the fluid in your seminal vesicles (which produce semen) to see if cancer has spread to these areas. 

Prostate cancer treatment

Many factors go into determining the right way to treat your cancer. Your doctor will use your age, as well as the type and stage of your cancer as part of the recommendations. If your cancer is diagnosed early, your doctor will also consider the likelihood that your cancer will grow and spread. Prostate cancers can grow very slowly, and some may need no treatment at all.

In addition to age, stage and likelihood of spreading, your oncology team may use biomarker tests as part of the planning process. These tests help identify genetic changes that can help you and your team decide on a prostate cancer treatment or tell your oncologist how well your cancer will respond to certain treatments.

Watchful waiting and active surveillance

Watchful waiting and active surveillance are two forms of observation used to monitor early-stage cancers that appear to be growing slowly or cause no symptoms. These cancers may not need treatment until they progress.

Your doctor may also recommend observation if you’re older and have other medical conditions.

  • Watchful waiting: With watchful waiting, you have regular appointments with your doctor to see if you develop symptoms or if any existing symptoms change. 
  • Active surveillance: Active surveillance also involves regular visits with your doctor, except that you will also have regular physical exams and testing to monitor your cancer. When these tests show concerning results, you’ll begin medical treatment. 

How to prevent prostate cancer

You may not be able to prevent prostate cancer. Many risk factors, such as your age and race, are beyond your control. Some research has found that certain dietary choices, such as dairy products, may increase your risk, but the evidence isn’t very strong.

If you are concerned about your cancer risk, you can take steps to live a healthier life, which will improve your overall health and may help you avoid prostate and other forms of cancer. You can: 

  • Eat a well-balanced diet. Eat more vegetables, fruit, whole grains and lean sources of protein, and consume less red and processed meat, sugar, and processed foods.
  • Exercise regularly. Aim for 150-300 minutes a week of moderate-intensity physical activity.
  • Limit alcohol intake. Men should have no more than two drinks per day.
  • Maintain a healthy weight. Work with your primary care provider (PCP) to determine what a healthy weight is for you.
  • Quit smoking if you smoke.
  • Stay on top of prostate cancer screenings. Your PCP can help you decide when to begin.
