All of our medical plans are adminstered by Baylor Scott & White Health Plan (BSWHP). If you are enrolled/planning to enroll in a BSW medical plan, be sure to link your employee account to your MyBSWHealth account for in-app access to the BSWHP.

What You Pay

Your cost for care depends on your medical plan, services received and where you go for care. With every plan, you get preventive care at no cost when you visit in-network providers.

The amount you pay per pay period depends on the plan you choose and the number of covered dependents. Premiums are deducted from your paycheck before taxes. If you change salary bands during the year, your premium will not be impacted.

Keep in mind the tobacco surcharge may impact the amount you pay in premiums.

Depending on which medical plan you choose, you'll have access to one or more provider networks.

 

Tier 1: BSW Premier HMO (NEW)

Best value!

Get high-quality care at the lowest cost from this complete network of:

  • 39,000+ primary care and specialty care physicians
  • 259 hospitals and ambulatory surgical centers
  • 270+ urgent care clinics
  • 9,000+ behavioral health providers through Lyra

It is included in all medical plans and the only network available for the SEQA and EQA plans (except in cases of urgent/emergency care or rare situations when an in-network provider is not available, and authorization is provided).

Tier 2: UnitedHealthcare (NEW)

Only available with the PPO and HDHP plans, gives you access to a national network of providers.

If you or a covered dependent live out of state or 40 or more miles from the nearest Tier 1 acute-care hospital you are eligible to receive the modified Tier 2 out of area coverage -80% vs. 50% after Tier 2 deductible for inpatient and outpatient services).
  • Out-of-state employees and covered dependents are automatically flagged for the out-of-area coverage (no action required).

  • Employees and dependents that live 40 or more miles from the nearest Tier 1 acute care hospital must contact BSWHP Care Connect Center at 844-843-3229 and request the out-of-area coverage prior to receiving care.

  • If you or your dependent had the out-of-area coverage activated but no longer live out of state or 40 or more miles from a Tier 1 acute care hospital, contact BSWHP to deactivate this coverage.

 

Tier 3: Out-of-Network

Only available with the PPO and HSA plans. Tier 3 out-of-network providers will always be the most expensive option.

Receive quality and convenient virtual care from a Baylor Scott & White provider (Tier 1) any time, using MyBSWHealth on a smartphone, computer or tablet. Learn how to schedule your virtual doctor visit today.

  • Video visits: Complete a face-to-face visit with your doctor using a camera-enabled device; or
  • eVisits: Complete a five-minute telehealth questionnaire about your symptoms and receive a response in less than an hour. 
Make sure your MyBSWHealth and BSWHP accounts are linked so your claim is processed accordingly.

Tobacco Surcharge

We are committed to you and your family’s long-term health. If you and/or a dependent enroll in a medical plan, you will be asked about your tobacco usage within the last 12 months. If you confirm the use of tobacco products, you will pay a $30 tobacco surcharge for each member who uses tobacco, up to a maximum of $60 per pay period.

To remove the tobacco surcharge, you or your dependents must complete a tobacco cessation wellness coaching or have your physician provide a certification that you have quit using tobacco products. Please note that enrollment in the Tobacco Cessation Program will not prevent the tobacco surcharge from being applied. The surcharge will be applied until you or your dependents have completed the program and submitted the Tobacco Affidavit to PeoplePlace, which is provided once you complete the Tobacco Cessation Program.
