The Choice Dental Plan gives you the flexibility to visit any licensed dentist, with the greatest savings when you use a Cigna Total Network provider. The plan focuses on preventive care and helps cover basic and major dental services after you meet the deductible. Annual plan benefits are capped at $1,250.

How it works

• Visit any licensed dentist and save more when using a Cigna network dentist.
• Preventive services are covered at 100% with no deductible.
• Pay a $50 individual deductible ($150 family deductible) before coverage begins for most basic and major services.
• After the deductible, you and the plan share the cost of covered services.
• Annual benefit maximum of $1,250.

The Choice Plus Dental Plan offers enhanced coverage and higher annual benefits than the Choice Plan. In addition to preventive, basic and major services, it includes orthodontia and implant coverage. Annual plan benefits are capped at $2,500.

The Dental Care Plan (DHMO) provides comprehensive dental coverage through a network of Cigna dentists. Instead of deductibles and annual maximums, most services are available with fixed copays listed in the Patient Charge Schedule, making costs more predictable.

• Choose a network general dentist when you enroll.
• Most care must be received through your selected network dentist.
• Specialty care is covered when referred to a participating specialist.
• Covered services are provided for fixed copays based on the Patient Charge Schedule.
• Preventive services such as exams, cleanings and many X-rays are covered at little to no cost.
